How to Become an Affiliated Agency?
The steps to becoming an affiliated agency with us are as follows:
Prospective Agency Application
Fill out and submit all requested details
Affiliation Agreement (PDF Format – for viewing only)
Review the Affiliation Agreement. Please note that if you are NOT requesting changes to the affiliation agreement you must complete the electronic version by using the link below. We are not accepting affiliation agreements via email or fax unless changes to the agreement are being requested. If you need to request changes to the affiliation agreement please reach out to us at email@example.com.
Agency Site Visit
Upon submission of the required paperwork, a member of our team will reach out to schedule a site visit. This meeting will include reviewing the Field Agency Information Guide and answer any questions you may have.
Please note: We will discuss the Field Placement Roles during our visit.
Once the agency has been approved and all details are finalized, you will receive a welcome e-mail from the Director of Field Education including access to our database and the Field Instructor Training.
