Many of the Field Education forms are submitted electronically on the agency database.  Students and field instructors will log in to the Field Web Based Database at https://www.alceasoftware.com/web/login.php to review and electronically sign the necessary forms.   A user name and password is needed to access this database.  Please email cswfield@osu.edu if you do not have a username and password or with any questions.  

Field Forms Definitions

Field Practicum Questionnaire
The field office will assign this form to you on the database.  You will complete this form to assist us in facilitating a placement for you.  It is very important that you complete the form in its entirety and by the deadline given. 
 
 
Learning Agreement/Evaluation
Completed by you and your field instructor. This is the document that guides your field experience. It is a living document and is meant to be referred to often and “tweaked” along the way. You must complete all assigned required activities and add activities specific to your placement agency.This document serves two purposes. First, it serves as the learning agreement which you should think of as the blueprint of what you’ll be doing in field. You will determine, along with your field instructor the learning activities that you’ll add to the required ones that are already listed. You will write in the target dates for each of these activities. The second purpose it serves is for your field instructor to evaluate your performance at the end of each quarter you are in field. Your field instructor will complete this by adding a rating for each learning activity in the “evaluation of practice behavior” column and for each competency overall in the bottom right corner.
 
 
 
 
 
Under Competency Two of the Learning Agreement/Evaluation is a reference for students to use an Ethical Decision Making Model.  Students may use a model that their agency has available or they can use the Reamer Model
 
Supervision Log
Completed by you, signed by your field instructor. As soon as you begin field, we suggest that you set-up a standing time for supervision with your field instructor. This form is to be completed each week so you should have 10 at the end of each quarter. You don’t have to indicate “progress towards competencies” in all of the competency every week, just the ones that were discussed in supervision.
 
Time sheet
Completed by you. Please be as accurate as possible when filling this out. Put the time you were there and try to fill it out each day to avoid inaccuracies or guess work. If you are in placement during finals week or over a break, you can use the space under week 1 or week 10.
  
Human Rights Activity Form
Completed by you. Each quarter you will spend 5 hours involved in an activity to advance human rights and social and economic justice. These hours count toward field and should be documented on your time sheet. This form is the assignment you will turn in after completing the 5 hours.
 
Human Rights Verification Form
Completed by you, signed by a representative of the event you attend.  This document is not on the database, but will be submitted to your field instructor for your evaluation at the end of each quarter.  Please click here to download this document.
