PhD Application Materials
New OSU graduate student applicants may elect to download this form for recommenders to complete or choose to have recommenders complete and submit the reference form and letters on-line.
Applicants who have previously been enrolled in the OSU Graduate School, should dowload the reference form for recommenders to complete.MSW-PhD applicants should submit the OSU and MSW reference forms. Visit the MSW application instructions page to download the MSW reference form.
Graduate Associate and Lecturer Application Form and Information
The Graduate Associate and Lecturer Form is optional for PhD and MSW-PhD applicants, but is required for those who would like to be considered for funding in the form of graduate associateships.
GRE
The GRE is required for all applicants. Please visit the GRE website to learn more about and register to take the GRE. http://gre.org/
