MSW-PhD applicants who may be eligible for the ASAP program should consult with the MSW Office concerning ASAP eligibility and any additional application materials needed for the ASAP program. Please include the MSW-PhD Letter of Recommendation form in addition to the OSU letter of recommendation form and letter.
New OSU graduate student applicants should use only the the OSU on-line application.

Graduate Associate and Lecturer Application Form and Information
The Graduate Associate and Lecturer Form is optional for PhD and MSW-PhD applicants, but is required for those who would like to be considered for funding in the form of graduate associateships.
New OSU Graduate School applicants should complete and upload this form when submitting the OSU on-line graduate application. Other applicants should submit it directly to the PhD program.
The GRE is required for all applicants. Please visit the GRE website to learn more about and register to take the GRE. http://gre.org/
